to form submission: • I understand that I am responsible for my own safety as a voluntary participant in group riding events sponsored by the Motorcycle Club. • I waive all claims against the J-Ville Riders Motorcycle Club, its officers, diplomats and members, the organizers of these events, and all participants in these events for any death, injury, harm, loss, and inconvenience or damage, I may suffer as a result of my voluntary participation in these events. • I consent to first aid and emergency medical treatment in the event of injury or illness. • I consent to allow my name and photograph to be published in print or electronic format in conjunction with any publicity associated with the club or these events. • I agree to obey all highway traffic laws and abide by all rules and instructions of the ride leaders. • I understand that the J-Ville Riders Motorcycle Club strictly forbids the operation of any motor vehicle during these events while under the influence of alcohol, drugs, or any medication that makes operation of a motor vehicle unsafe. By submitting this form, I acknowledge that I have read, understood, and agree to the above release. I agree to abide by the rules and regulations of the club. I certify that I am at least 18 years of age or if under 18 a parent/guardian signature is required.
